§第五組第一次case討論會§

 時間:4/8(三)考完醫倫後

 地點:立夫103(考試的教室之後都有人要上課，所以麻煩先考完的同學到這間教室等囉!!)

 內容:報告呈現方式、工作分配、工作行程訂定

 說明:請大家務必都要到!!我已經擬好幾個方向和分工的草稿，

      大家只需要提供改正意見以及表決，所以不會耽誤大家太多時間的!

年齡：28歲    職業：房屋仲介         學歷: 大學畢業     婚姻狀況: 未婚

主要症狀

Weight loss by 7 kg during the past 3 months

簡單病史

This 28 year-old woman, a real estate agent with unremarkable
medical history, reported that she had lost weight by 8 kg during the past 3
months. Besides, she became anxious, irritable and heat-intolerable at the
same time, and she also presented with hand tremor, diaphoresis and menstrual
irregularity recently.

理學檢查

a.  Appearance: cooperative, anxious and restless, well-nourished, proptosis
of eyes

b.  Pulse rate: 108 per minute (regular bounding pulse), blood pressure:
130/60 mmHg, respiratory rate: 20 per minute

c.  Moist and warm skin

d.  Fine hand tremor, without pretibial pitting edema

e.  Inspection of the neck: a movable grade 2 diffuse goiter,
    Palpation: of the neck: elastic, no palpable nodule
    Auscultation of the neck: audible bruit

f.  Chest: normal chest wall and expansion, clear breath sound

g.  Abdomen: flat, hyperactive bowel sound, soft, no tenderness, no
    hepatosplenomegaly

實驗室檢查

    Hb: 14.0 g/dL,         Cre: 0.9 mg/dL,    GPT: 34 U/L,
    Albumin: 4.0 g/dL      TSH: 0.001 μIU/mL free T4: 3.01 ng/dL
    TSH receptor antibody: 65%
    EKG: sinus tachycardia
    Chest X-ray: normal heart size, unremarkable finding

c.  就本個案而言, 在做理學檢查和實驗室檢查之前, 您想要再詢問哪些病史呢以增加您
    對她的了解和診斷 ?

d.  本個案的理學檢查給了您哪些資訊呢? 您考慮還要再做哪些理學檢查呢 ?

e.  就本個案而言, 您認為哪些可能的原因會造成她的體重減輕 ? 為什麼 ?
