cardiovascular disease in the past. He received a general health examination
two years ago and no significant abnormality was noted at that time. However,

kg, in past 6 months was noted. He denied dysphagia, gastralgia, diarrhea, or
change of bowel habits during the past six months. He denied any other
discomfort, either.
