70歲王媽媽患有高血壓和第二型糖尿病二十幾年，身體檢查發現下肢血液循環不佳。

其腳踝及上臂血壓測量如下。右手血壓160/80mmHg；右下肢血壓40/20mmHg；左手血壓

100/60mmHg；左下肢血壓50/38mmHg，下列敘述何者為誤?


(A) Right ankle/branchial index：0.25

(B) Left ankle/branchial index：0.50

(C) 右下肢血管堵塞程度 > 左下肢

(D) 其左手動脈可能亦合併有狹窄之可能

先看臨診共筆 no.7 嘉義四仙女  P.41  Ankle-Brachial Index (ABI)的公式計算

Right ABI：(higher right ankle pressure)/(High arm pressure)

Left  ABI：(higher left ankle pressure) /(High arm pressure)

其中的 Pressue 代表 SBP

High arm pressure = 左、右手較高的數值

題目：右手血壓160/80mmHg、左手血壓100/60mmHg → High arm pressure = 160

所以~~ Right ABI = 40/160 = 0.25

       Left  ABI = 50/160 = 0.3125

ABI 越低代表阻塞越嚴重

所以選項 (C) 右下肢血管堵塞程度 > 左下肢 →正確

    選項 (D) 其左手動脈可能亦合併有狹窄之可能

             正常而言 右手血壓比左手血壓高約5-10mmHg (網路資料)

             因此選項 D 為正確

             註：心循共筆No.4  P.51 如果左右手壓差超過20可能有主動脈剝離

回頭看選項~

答案應該包含Subjective and objective finding, management, evaluation(評估)

選項只剩下 D、E

E選項的 Cost → Progress note(病程紀錄)，應該跟Cost(花費)沒什麼關係

另外 D選項中的 Impression(臆診) 請看 P.13 assessment的部分第二點

diagnoses

如果診斷結果是不確定的，就列出問題及其餘可能的診斷

水平姿勢 → 站立
