[文法] 過去式及過去進行式的差別

各位好,想請教一題文法
這是基測考題的句子,答案是 was having
My mom ___ a hard time when the doctor said I was too big to be pulled out.

但想請問如果使用過去式 had 是否正確,另外語意與 was having 有何差別? 謝謝!
