He failed in passing the test. (fail不及物，後接"in＋受詞")
他無法通過考試。
(vt)失敗
He failed to pass the test. ("to pass the test"為fail的受詞)
他無法通過考試。
(vt)使不及格
He failed the test.
= The test was failed by him.
= He was failed in the test.
他使考試不及格 = 他被使不及格在考試上(被動態)
第一句：the boy failed the test 文法沒錯。
可以改成被動態：the test was failed by the boy
第二句：the boy was failed to the test，
其中 was failed 文法也對！
但是 你第二句的介系詞應該是用in，
*也就是，the boy was failed in the test (by his teacher)
*來自於：His teacher failed the boy in the test!!
