government issued identification (e.g. a driver's license or passport)
to 1-952-681-3681 Attn: GMAT Program Coordinator that shows
documentation of your new name. Please include a cover letter with an
explanation of why your name needs to be changed and your contact
information. Or, you may send those documents via scan copy to this
e-mail address and we will assist you in correcting your name.
