The reason why the answer to the first question is
"you don't have to" is because that's what SOUNDS right.
What sounds right is what people say often.
If what sounds right to native speakers doesn't sound right to me,
it means that this is a new expression to me
and I am going to learn it today,
so that next time when I see this expression,
it wouldn't sound foreign to me anymore.
See, naturally English is all about how it sounds.
"How many times a week do you play baseball?" sounds the best to me.
"How often do you play baseball IN a week?" sounds ok.
"How often a week do you play baseball?" doesn't sound so bad
and I wouldn't say it's wrong.
