申請適婚能力證明須知REQUIREMENTS FOR APPLYING LEGAL CAPACITY

國人應準備文件DOCUMENTS REQUIRED FOR TAIWANESE NATIONALS:
（國內所發戶籍謄本及單身宣誓書無需翻譯為英文且無需外交部與菲律賓駐台辦事處認證
）
        1.      申請書.Duly accomplished Application Form
2.      最近三個月戶籍騰本.Household Certificate (Original issued w/in last 3
months.)
3.   經台灣地方法院公證之單身宣誓書.Affidavit of Single Status duly notarized
by Taiwan District Court. (Original)
4.      護照. Passport (Original & a photocopy of all pages used.)
5.      身份證. Taiwan ID (Original & a photocopy)
6.      最近三個月之健康檢查.Medical Certificate issued within the last three
months by R.O.C. Hospitals accredited by R.O.C. Department of Health.

7.      If the Taiwanese National is born in the Philippines, attach Birth
Certificate and Certificate of no records of marriage (CENOMAR) secured from
the National Statistics Office, and duly authenticated by the Office of the
President and Department of Foreign Affairs.  Affix Documentary stamp

菲律賓人應準備之文件DOCUMENTS REQUIRED FOR FILIPINO FIANCE OR FIANCEE
1.      護照. Present and Previous Passports.  (Original & a photocopy of all
pages used.)
2.      出生證明須經菲律賓總統府及外交部驗證.Birth Certificate secured from the
National Statistics Office, and duly authenticated by the Office of the
President and Department of Foreign Affairs. Affix Documentary Stamp.

*NOTE: Birth certificate registered after 12 years old, or document with
Correction of entry exceeding 3 items would not be accepted. Late birth
registration done before 21 years old may seek an affirmative court decision
on the assumption of identity for reconsideration.
3.      單身證明須經菲律賓總統府及外交部驗證. Certificate of no records of
marriage (CENOMAR) secured from the National Statistics Office, and duly
authenticated by the Office of the President and Department of Foreign
Affairs.  Affix Documentary Stamp. (Original )
4.      如在菲律賓有前婚姻紀錄須提供結婚紀錄證明、註銷證明及死亡If one of the
parties have previous marriage in the Philippines, attach Advisory on
Marriages,  Marriage Certificate with remarks of annulment and Death
Certificate (if spouse is dead) secured from the National Statistics Office,
and duly authenticated by the Office of the President and Department of
Foreign Affairs.  Affix Documentary stamp. (Original & a photocopy.)
5.      最近三個月之健康檢查須經菲律賓總統府及外交部驗證.Medical Certificate
issued within the last three months from Accredited Hospitals and duly
authenticated by the Office of the President and Department of Foreign
Affairs. Affix Documentary Stamp. (Original & a photocopy)
雙方應備事項 TO BE ACCOMPLISHED BY BOTH PARTIES
1.      基本資料表.Duly accomplished Personal Data Sheet (one copy for each party.)
2.      交往證明.雙方如經人介紹或透過網路交友認識需提供至少持續一年以上之書信或電子
郵件等往來文件. Evidence of Communication, e.g. letters, pictures, telephone
bills or any other verifiable evidence. Those who are matched through
personal introduction or through internet shall present hard copy of direct
communication and substantial correspondence for at least one year. E.g.
letter, email etc. (Original & a photocopy.)
Reminder: Republic Act No. 6955 declares unlawful the act of matching
Filipino women for marriage to Foreign Nationals on a mail-order basis or
through personal introduction and other similar practices.
3.      無業或收入不足者須提供財力證明.If one of the parties is unemployed or has
low income, attach financial documents or duly notarized affidavit of support
executed by supporter..
4.      照片. Recent photo of both parties 2X2 with white background.
