◎符合以下其中一項：
(1)糖尿病、氣喘、COPD、SPAF、高血壓患者
(2)家中肺癌病患照顧者
車馬費2500元
時間
G1 12/4(一) 15:30~18:00
G2 12/5(一) 18:30~21:00
G3 12/6(二) 15:30~18:00
G4 12/6(二) 18:30~21:00
座談會含報到時間約2.5小時
須提供患者相關病證證明
須配合座談會前安裝使用測試疾病管理工具(APP)兩週
