[問題] Tennis Warehouse 要求要信用卡授權資訊

板友們好，昨天晚上在 Tennis Warehouse 訂購了兩支球拍，

加上運費要 274 美元，

原本以為只要像許多網站提供卡號之類的資訊就好，

但是 Tennis Warehouse 卻寄來了一封 E-mail，要求要提供親筆簽名之類的資訊，

全文如下：

Thank you for placing an order with Tennis Warehouse!

Our online ordering system is very secure and ensures that no one can tap
into your transmission and use your credit information. However, this does
not stop people from entering false information.

This authorization form is a procedure to confirm that the authorized
cardholder has made the order. This is an extra security measure we require
to protect you, the cardholder, and Tennis Warehouse, from credit card fraud.

The form below is issued to all new customers and will not be required for
future orders. (* Previous customers using a shipping address and/or card
number different to the one we have on file may be asked to complete a new
authorization form.)

Required Information:
The following information will need to be e-mailed, faxed, or sent via
postal mail before an order can be processed:

1. The name, address and telephone number of the bank issuing your credit
card
2. A copy of your credit card (both sides)
3. A copy of your personal identification card or driver's license
4. The signed credit card authorization form below

AUTHORIZATION FORM:

Please initial and complete the following:
____ I understand that it is my responsibility to consult with my local
government agency to determine any applicable duties and taxes associated
with my order, and the extent of those fees.
____ I understand that I will be responsible for all applicable duties and
taxes upon delivery of my order.
____ I accept that if I refuse the delivery of my order, I authorize Tennis
Warehouse to charge my credit card for the cost of the applicable duties and
taxes, as well as the return delivery charges back to Tennis Warehouse.
____ I understand that I will be responsible for any return shipping costs
associated with my order.

裡面大概就是說，要我提供信用卡正反面的副本、證件或駕照副本，

以及後面這個格式的表格，

這個表格還要求要填入我的親筆簽名、信用卡發卡銀行名稱、銀行地址、銀行電話等，

可是我是在台灣，證件難道是要給台灣的身分證或是護照嗎？

而銀行的地址和電話等資訊，真的就填入信用卡發卡銀行的資訊就好了？

其實最令人在意的，是要提供信用卡的影本，

不知道在傳輸的過程中會不會出了什麼意外......

而且我信用卡簽名是中文的，傳回去的也是簽中文就好嗎？

請問有板友有這個網站的購物相關經驗嗎？

另外請問一下，因為這個價格超過 3000 台幣，

是不是寄過來時海關一定會再課 10% 的關稅還有 5% 的營業稅呢？
