Academic Researchers/Advisor Letter: Applicants performing academic research in the U.S. must also provide a letter from their academic institution advisor(s) answering the following questions.
All four questions MUST be answered briefly. If the question is not applicable, please answer with N/A.
1. What are the goals and practical applications of the Applicant's research?
2. What are the source(s) of funding for the Applicant's research?
3. What export-controlled technologies and/or information will the Applicant have access to during his/her research?
4. What U.S. government projects will the Applicant participate in or will have access to during his/her research? Please state whether non-U.S. citizens have access to the project.

CURRICULUM VITAE (CV): Include a short summary of your company’s products/services, and business activities in your home country and/or in the U.S.
CONTACT INFO IN THE US: List the street/email addresses, names, and phone numbers of your U.S. points of contact at, friends/family residence, organizations, institutions, and/or companies that you intend to visit in the U.S. (For academic institutions or conferences, provide the name of an academic contact, not the name of the foreign student advisor.)
CUSTOMER NAME LIST: List any customers of your company with their names, address, and phone numbers.
EXPORT LICENSE NUMBER: Provide export license number information (or, if pending, an export license application number) covering the activity in the U.S.
INVITATION LETTER: Provide a letter from your U.S. sponsor, including the purpose of visit and/or activities to be engages in the U.S. and conference invitation.
PREVIOUS TRAVEL RECORDS: List all your travel for the past ten years in detail with the dates of travel, country name, and purpose of travel.
PUBLICATION LIST: Include a complete list of your academic and professional publications.
EMPLOYMENT DOCS (Petition Based H/L/O/P visa applicants): Provide a letter prepared by company or attorney summarising facts of company and how applicant fits to company structure.
RESEARCH/THESIS ABSTRACT: Provide a short summary of the research thesis from your advanced degree of education. If you plan to present your research at a conference in the U.S., also include the research abstract for your presentation.
SHORT RESUME: Please refer to attached document in MS word format.
