[北美] [E-2] 離境聲明書

想請問一下，關於附件C欄中的

"申請人簽署的聲明書，表明他/她打算在E身分結束時離開美國"

這個聲明書有格式規定或範本嗎？


還是只要短短的用英文寫

  "I intend to depart i intend to depart the country, U.S.A,

right after my E-2 VISA expired."

然後底下簽名就好？
