這個問題當然不只是醫學上的問題

很多牽扯到政治正確與否

但既然在這裡醫學生版

也許我們可以單純以醫學角度來看

相信很多人都有在看 NEJM 當興趣

2017年01月有一篇相關的文章
Rethinking the Ban — The U.S. Blood Supply and Men Who Have Sex with Men

提到近35年來最大的兩個進展
1. Progressive shortening of the window period
HIV RNA 空窗期縮短到 9 ~ 14 天

2. Improvements in the test's sensitivity
sensitivity of HIV testing
97.3% in 1985 -->  100%  in  2016

使感染機率降低 risk of HIV transmission
1 in 1.5 million

而這與自我管控(self-deferal)無關


很快就有不少回應
Changes in France’s Deferral of Blood Donation by Men Who Have Sex with Men

N Engl J Med. 2017 Apr 13;376(15):1485-1486.

根據法國統計(French national surveillance system for blood donors,2011-2015)
108 / 142 donors (76%) who were found to be HIV-positive were men.
 82 / 108 有資料可回朔
 49 /  82 (60%) reported having had sex with men.

 20 /  22 (91%)HIV nucleic acid–positive, antibody-negative donors were men
 11 /  20 were MSM


在香港也有篇論文
Low compliance of men having sex with men with self-deferral from blood
donation in a Chinese population

Blood Transfus. 2014 Apr; 12(2): 166–171.
doi:  10.2450/2013.0103-13

Over a 6-week period in 2012, 250 MSM,
three-quarters of whom were aged between 16 and 30 years old

 119/ 250 (47.6%) 曾捐血.
  90/ 119 (75.6%) 是重複捐血者
約     1/3 每3至6個月捐血一次

但與非捐血者比較
a higher proportion of blood donors had had anal sex (OR:2.39)
and multiple partners (OR:1.53) in the preceding year
while their condom usage rate was similar

non-compliance的原因
(1) belief of their right to donate (53.8% giving this as the main reason)
(2) confidence about the safety of their blood at the time of donation
(43.7% as the main reason)


這兩篇都顯示一件事

自我管控機制是失效的....

但是否因此要全面撤除自我管控

我認為不適合

與其全部在檯面下隱藏

反而翻到檯面上可以藉此收集相關數據與管理

至於血要不要用 如何用

那又是另一件事了
